“Based on record review and interview, for one of two residents sampled, the manager failed to ensure medication was administered to a resident in compliance with a medication order.        Findings include:       1. A review of R2’s medical record revealed a medication order for “Clonidine HCL 0.1 MG Take 1 tablet by mouth daily. Hold for HR<60 or SBP<100.”        2. A review of R2’s electronic medical record for April 2026 revealed a section used for documenting the administration of “Clonidine 0.1 mg,” with instructions reading “Give 1 tab PO Daily (Hold for HR<60 OR SBP<100).” The record reflected Clonidine was administered each day between April 1 and April 28, 2026, including April 4, 11, 14, and 21, 2026.       3. A review of facility documentation revealed a calendar documenting R2’s heart rate and blood pressure for April 2026. The documentation reflected R2’s heart rate or systolic blood pressure was below parameters on April 4, 11, 14, and 21, 2026, indicating the medication should not have been administered on those days.       4. In an exit interview, the findings were reviewed with E1, and no additional information was provided.”
