STATE OF WASHINGTON DEPARTMENT OF SOCIAL AND HEALTH SERVICES Aging and Long-Term Support Administration PO Box 45600, Olympia, WA 98504-5600 July 6, 2023 ELECTRONIC-FACSIMILE Administrator Aegis of Bellevue 148 102nd Ave SE Bellevue, WA 98004 Assisted Living Facility License #2491 Licensee: 102 Ave Bellevue LLC IMPOSITION OF A CIVIL FINE Dear Administrator: On June 29, 2023, the Department of Social and Health Services (DSHS), Residential Care Services completed a follow-up visit at your facility. This letter constitutes formal notice of a civil fine on the license for your assisted living facility, also known as Aegis of Bellevue, located at 148 102nd Ave SE, Bellevue, by the State of Washington, Department of Social and Health Services. These actions are taken under the authority granted pursuant to Laws of 1998, Chapter 272 and RCW 18.20.190. The civil fine on the license is based on the following violation of the RCW and/or WAC as described in the attached Statement of Deficiencies (SOD) report dated June 29, 2023. Civil Fine WAC 388-78A-2730 (1)(a)(b) Licensee's responsibilities. $300.00 The licensee failed to implement their policy for Respiratory Protection Program (RPP) for eleven staff who have direct contact with residents. This failure placed 78 residents at risk of contracting and spreading a potentially life-threatening disease. This is an uncorrected deficiency previously cited on March 08, 2023. NOTE: This is the violation, which resulted in the fine; see the attached Statement of Deficiencies for any additional violations.
