“Eight resident rooms in the Darlington Neighborhood were observed to have toiletries unlocked, unattended and accessible to residents, including items with manufacturer's warnings indicating poisoning risk (antiperspirant deodorant, mouthwash, toothpaste). Residents in this unit have been assessed as not capable of safely using or avoiding poisonous materials.”

“During a medication administration observation, a staff member did not wash or sanitize their hands between administering medications to different residents.”

“Emergency telephone numbers including the nearest hospital and fire department were not posted on or by the telephones in certain rooms in the Memory Support Unit.”

“The kitchen refrigerator in the Darlington Neighborhood was in disrepair and leaking water onto the floor, with towels placed inside to absorb the water.”

“Staff Person A did not receive orientation on their first work day covering evacuation procedures, staff duties during fire drills and emergency evacuation, designated meeting places, smoking safety procedures, fire extinguisher location and use, smoke detectors and fire alarms, and telephone use for emergency services.”

“One resident's medical evaluation did not include a general physical examination by a physician, physician's assistant, or nurse practitioner, and did not include medical information pertinent to diagnosis and treatment in case of an emergency. Another resident's medical evaluation did not include special health or dietary needs and body positioning and movement stimulation information.”

“One resident's most recent medical evaluation was not completed within the required annual timeframe.”

“The assessment for one resident is missing a determination regarding irritability, judgement, agitation, aggression, and hallucination, and does not indicate if the resident has a need for behavioral care services or how this need will be met.”

“An accumulation of lint was found in the lint trap of the second floor dryer in The Hickman Building, violating the requirement that lint shall be removed from the lint trap after each use.”

“The home routinely held fire drills in the last week of the month on specific dates (02/22/23, 03/23/23, 04/28/23, 05/23/23, and 06/23/23), violating the requirement that fire drills be held on different days of the week and at different times, not routinely.”
