Based on observation, interview, and record review, it was determined the facility failed to ensure the kitchen was maintained in accordance with the Food Sanitation Rules OAR 333-150-000. Findings include, but are not limited to: Observations of the facility kitchen, food storage areas, food preparation, and food service on 12/18/23 revealed: * Soap and paper towels were not available at the hand washing sink; * Splatters, spills, drips, and debris build-up was noted in the drawers, cupboards, microwave, and reach-in refrigerators and freezers; * Damage to interior shelving, cupboard doors, and drawers creating un-cleanable surfaces; * The glass of the oven door was broken; * Freezer in garage with approximately three inch build-up of ice; * Food stored in garage was on the floor, on a table with craft supplies, and on shelving in same area as vacuum and cleaning supplies. * Eggs stored above ready to eat foods; * Packaged raw hamburger on towel in refrigerator with ready to eat foods in direct contact; * Opened undated packaged foods; * Thermometers, found stored in drawer, visibly dirty; and * Small diameter probe thermometer to measure thin foods was not working. Universal Workers, who provided care to residents, were observed to not use aprons during food plating and service. Staff failed to have beards restrained. Staff were observed to not change gloves between tasks during food preparation and service, and failed to wash or sanitize hands when entering the kitchen. The food handling and storage concerns, and the areas in need of cleaning and repair, were reviewed with Staff 1 (Executive Director) on 12/18/23. She acknowledged the findings. Soap and Paper towels are available at the handwashing sink. *Splatters, spills, drips, and debris and build up noted to be in the drawers, cupboards, microwave and reach -in refrigerators and freezers have been deep cleaned. *Damage to the interior shelving doors and drawers have all been cleaned and has a cleanable surface. *The Glass door on the oven has been replaced with a new one. *The Freezer in the garage has been defrosted and free of ice build up. *The food stored in the garage on the floor has been removed and there are no cleaning supplies near the pantry. *All eggs have been moved to the bottom shelf of the refrigerator. *The Packaged raw meat was removed from the hand towel and all raw meat will be stored in a drawer labeled "Raw Meat" *All opened and undated packaged foods were dated with the proper open date on them. *The thermometer in the drawer was replaced and a cleaning system has been implemented. *Universal Workers who will be serving food, prepping, preparing and serving will have aprons on at all times when in the kitchen. *Staff will wear beard nets when in the kitchen if has facial hair. *Staff will change gloves in between preparation and serving. *Staff will also wash and sanitize hands prior to entering kitchen and inbetween tasks. 2.The kitchen binder with task sheets for cleaning and auditing the above issues are in place. *All staff to be retrained on deep cleaning the kitchen and proper food handling everytime they are assigned to the kitchen duties. *Hair and beard nets will be worn in the kitchen. *All surfaces will be monitored for cleanliness and ensuring cleanable surfaces. *The freezer will be checked weekly to ensure ice has not built up. *A new thermometer has been purchased and properly stored with instructions on cleaning and proper storing. *Refrigerator has been labeled on the shelf where eggs are to be properly stored. *Labels have been purchased and placed with a marker in the menu drawer to properly date opened food. *Cleaning supplies have been relocated away from food storage in the garage. 3.Daily spot checks and weekly auditing will be done 4.The Assistant Executive Director and/or Executive Director will be responsible for monitoring.
