“Based on record review and interview, the manager failed to ensure a caregiver provided assistance with, or directed a resident’s personal hygiene according to the resident’s service plan, for one of three residents sampled. Findings include: 1. A review of R1’s medical record revealed a service plan which included a section titled “Shower Directed Care,” which read “[R1] will get a bed bath twice weekly due to safety issues…SHOWERS WILL BE DONE BY [R1’s hospice provider].” The service plan indicated R1 was to receive bed baths twice weekly, on Tuesdays and Saturdays. Further review revealed a “Care Tracking Sheet,” used for documenting services and activities of daily living for May 2025. The tracking sheet contained documentation indicating R1 was not showered on Saturday, May 3, 10, 17, 24, or 31, 2025. 2. In an interview, E1 advised R1’s hospice provider assisted R1 with showers; however, R1’s hospice provider did not come on Saturdays. E1 said R1’s shower schedule was not amended to accommodate R1’s hospice provider, and the caregiver did not assist R1 with bed bath services on Saturday. E1 agreed the caregiver did not assist with R1’s personal hygiene twice weekly as outlined in R1’s service plan.”
