Based on record review and interview, the manager failed to ensure a medication administered to a resident was administered in compliance with a medication order, for one of two residents sampled who received medication administration. Findings include: 1. A review of R1's medical record revealed a service plan, dated November 13, 2024, for directed care services including medication administration. 2. A review of R1's medical record revealed a list of medication orders, dated December 27, 2024, which included the following: - "Atenolol 50 mg tablet, Take 1 tablet (50 MG) by oral route once daily hold for SBP

Based on observation and interview, the manager failed to ensure foods requiring refrigeration were maintained at 41\'b0F or below. Findings include: 1. During an environmental tour of the facility, the Compliance Officer observed a refrigerator in a storage room adjacent to the kitchen. The refrigerator contained foods requiring refrigeration, such as applesauce, margarine, milk, cream cheese, and orange juice. However, a thermometer in the refrigerator read 48\'b0F. 2. In an interview, E1 acknowledged foods requiring refrigeration had not been maintained at 41\'b0F or below.
