Based on record review and interview, the health care institution's chief administrative officer failed to ensure that the health care institution established, documented, and implemented tuberculosis infection control activities for one of three personnel reviewed. The deficient practice posed a potential TB exposure risk.  Findings include: 1. A review of E3's personnel record revealed documentation of a 2-step Mantoux Skin test with "negative" results; however, there was no documentation of the TB Risk Assessment/Signs & Symptoms Screening. Based on E3's date of hire, this documentation was required. 2. In an interview, E1 acknowledged E3's personnel record did not contain the TB Risk Assessment/Signs & Symptoms Screening form at the time of the inspection.

Based on record review and interview, the governing authority failed to ensure compliance with A.R.S. § 36-411.C.1 for three of three personnel reviewed. The deficient practice posed a risk as required information could not be verified for personnel to determine a person's fitness to work in the assisted living home. Findings include: 1. A.R.S. § 36-411.C.1 states, "C. Owners shall make documented, good faith efforts to: 1. Contact previous employers to obtain information or recommendations that may be relevant to a person's fitness to work in a residential care institution, nursing care institution or home health agency." 2. A review of E1's, E2's, and E3's personnel records revealed documentation of personal reference checks; however, there was no documentation of good faith efforts to contact previous employers to obtain information or recommendations that may be relevant to a person's fitness to work in the assisted living home. 3. In an interview, E1 explained that E1 personally knew E2 and knew of E2's fitness to work from witnessing it at other homes. E1 was also confident in E2's fitness to work. E1 had documented personal reference checks for both E2 and E3, but was unaware of the requirement to document reference checks of past employers. E1 acknowledged that reference checks of past employers had not been documented in E1's, E2's, and E3's personnel records.

Based on R2's date of admission, this documentation was required. 2. In an interview, E1 explained that R2 had come to the facility with the Mantoux Skin test already conducted the month prior, and that E1 was unaware of the need to complete the TB Risk Assessment/Signs & Symptoms Screening at the time of admission, but would get it done immediately. E1 acknowledged R2's medical record did not contain the TB Risk Assessment/Signs & Symptoms Screening form at the time of the inspection.
