“A physical altercation occurred between two residents in the hallway. During the altercation, resident #1 grabbed resident #2's right wrist and slammed resident #2 into the wall, causing a red mark with fingerprint imprints. This is a repeat violation from 7/6/2022.”

“At 10:52 AM, resident #2's private bathroom had no paper towels, mechanical air blower, individual cloth towels, or other sanitary means of hand drying. At 11:06 AM, resident #3's private bathroom had the same deficiency.”

“The right corner of the toilet tank lid was broken off and missing from the toilet in resident #4's private bathroom.”

“The freezer section of the refrigerator/freezer in the Berry Ridge kitchen was 12 degrees Fahrenheit at 10:16 AM and 2:45 PM. The freezer section of the refrigerator/freezer in the studio activity room kitchen was 12 degrees Fahrenheit at 10:46 AM and 2:41 PM. No thermometer was present in the refrigerator section of the studio activity room kitchen refrigerator/freezer.”

“Resident #2's most recent medical evaluation does not include resident #2's blood pressure or ability to self-administer medications, as these sections are blank.”

“Resident #5 is prescribed Atropine Sulfate 1% drops to be placed 2 drops under tongue every 1 hour as needed; however, this medication was not available in the home for administration. This is a repeat violation from 1/8/2024.”

“Medication Administration Records (MARs) for residents in Boathouse and Garden units were unlocked, unattended, and accessible on top of medication carts in the hallway, violating resident record confidentiality requirements.”
