“The True commercial freezer in the kitchen was measured at 15°F at 1:43pm and 11°F at 3:42pm. The upright Frigidaire freezer was measured at 12°F at 1:45pm and 9°F at 3:40pm. Both freezers failed to maintain the required temperature of 0°F or below for frozen food storage.”

“Three resident-to-resident incidents involving physical altercations resulted in injuries: forcible hand grab causing bruising, forcible grab and hit causing skin tear, and physical altercation causing scratches and bruising to multiple residents.”
