“Poisonous materials were not properly secured. A laundry room in the Pearl Neighborhood was unlocked and unsupervised with four containers of laundry detergent labeled "keep out of reach of children" in an unlocked cabinet, accessible to residents who are not assessed as able to recognize and use poisons safely.”

“Fire drill records contained discrepancies between the number of residents documented as being in the home and the number documented as being evacuated on six separate occasions. The documentation did not accurately reflect evacuation compliance.”

“Resident-on-resident physical abuse. Multiple incidents documented: (1) Resident mistook another resident for spouse, squeezed their hand causing dark red marks and indentations; (2) Resident grabbed another resident's head and forcibly turned it toward television, then hit them on nose; (3) Resident scratched another resident's hand causing 1.5 inch skin tear; (4) Resident struck another resident three times on upper arm causing redness; (5) Resident slapped another resident across cheek and ear while passing in dining room.”

“A resident engaged in a physical altercation with another resident, striking him and causing a bite wound on the resident's left hand that required cleaning and dressing. This constitutes abuse and neglect of a resident.”

“A staff member was re-hired after an absence of more than two years without a current Criminal History Background Check (CHBC). The facility only had results from a CHBC completed in 2014 and could not locate documentation of a background check from the October 29, 2018 rehire date.”

“The facility failed to conduct a fire drill during sleeping hours (11pm–7am) within the required 6-month interval. The last fire drill conducted during sleeping hours was on 3/10/2023.”

“An alleged case of verbal abuse was reported to the home where Staff Member A yelled at Resident 1 and made inappropriate comments in front of other residents. This allegation of abuse was not reported to the Local Area Agency on Aging.”

“Staff Member A yelled at Resident 1, made inappropriate comments, and appeared to threaten Resident 1 in front of other residents. Staff Member A also threw a utensil at Resident 1. This violated the requirement that residents be treated with dignity and respect.”
