Multiple incidents documented: (1) Resident mistook another resident for spouse, squeezed their hand causing dark red marks and indentations; (2) Resident grabbed another resident's head and forcibly turned it toward television, then hit them on nose; (3) Resident scratched another resident's hand causing 1.5 inch skin tear; (4) Resident struck another resident three times on upper arm causing redness; (5) Resident slapped another resident across cheek and ear while passing in dining room.
