“The last fire safety inspection and fire drill observed by a fire safety expert was conducted on 3/27/25. The previous inspection and drill was conducted on 2/22/24, indicating a gap exceeding the annual requirement.”

“Resident 1 is prescribed Tobramycin eye drops – 2 drops in right eye at hour of sleep. On 5/9/25, 2 drops were administered in the left eye instead. Additionally, Resident 2 is prescribed Prednisolone Eyedrop to left eye BID; on 5/14/25, the morning dose was administered in both eyes instead of the left eye only.”

“Housekeeping closet containing cleaning supplies including Lysol disinfectant cleaning spray was unlocked, unattended, and accessible to residents in the 200 halls. Not all residents have been assessed as capable of recognizing and using poisons safely. This was a repeated violation from 11/2/22.”

“Unlabeled and undated ziplock bag containing a yellow powder-like substance and a small container of brown liquid were found in the refrigerator located in the home's activity room.”

“A small white box containing combustible or flammable materials was stored behind the gas-powered hot water heater in the maintenance room, in violation of requirements that such materials not be located near heat sources or hot water heaters.”
