“A resident was administered a tablet prescribed for a different resident. The medication error involved giving one resident another resident's prescription medication.”

“The facility failed to follow prescriber's orders for multiple residents. Three prescribed medications (an extended-release capsule for anxiety, a topical ointment for temple area, and a topical paste for groin/buttock area) were not administered because the medications were not available in the home. This is a repeated violation.”

“A resident's annual assessment was not completed on schedule; the most recent assessment was overdue at the time of inspection. The resident has since been discharged from the facility.”

“There are no emergency telephone numbers, including the nearest hospital and fire department, posted on or near the telephone in the Third Floor Solarium/Sunroom.”

“Resident home contract dated July 6, 2022 was signed by the home but not by the resident. This is a repeated violation from 8/2/22.”

“On 10/31/23 at approximately 10:30 AM, a bottle of Ultra Concentrated Fabric Softener was found unlocked, unattended, and accessible to residents in the Third Floor Secure Care Unit laundry room. On 10/31/23 at approximately 11:00 AM, a bottle of Odoban Disinfectant Spray and a bottle of Febreze disinfectant spray were found unlocked, unattended, and accessible to residents in the 3rd floor Secure Care Unit public bathroom. Not all residents of the Secure Care Unit have been assessed capable of recognizing and using poisons safely. This is a repeated violation from 8/2/22.”

“The freezer in the Third Floor Secure Care Unit contained a bag of frozen waffles which was unsealed and open to the air, in violation of food storage requirements.”

“On 10/31/23, there was an approximate 1/4 inch accumulation of lint in the lint trap of the dryer on the Third Floor Secure Care Unit, in violation of fire hazard prevention requirements.”
