“A resident was observed hitting another resident with a closed fist and pushing them to cause a fall. The home did not report this incident to the Department until 3:30 PM, failing to meet the 24-hour reporting requirement.”

“A resident's most recent annual medical evaluation did not include the resident's health status or cognitive functioning as required.”

“A discontinued prescription medication was found in the home's medication cart. The prescription had an expiration date that had passed, yet the medication remained in the cart. This is a repeated violation from 9/13/23 et al.”

“A resident was observed with a swollen left hand, wrist, and arm with multiple wrist fractures revealed by x-rays, but no falls or incidents were reported to account for the injuries. The incident was reported to the Department and local area agency on aging, but was not reported to the Pennsylvania Department of Aging or local police as required.”

“A staff person hired did not complete required training within 40 scheduled working hours, specifically training in the home's emergency medical plan and reporting of reportable incidents and conditions.”

“Enabler bars in rooms 214 and 202 were not secured to the bed frame; they were attached to a wooden plank that slipped between the mattress and box spring, posing an entrapment risk.”

“A red container of sink and surface cleaner/sanitizer was observed on the 4th floor pantry food prep table next to hamburger buns, storing poisonous material adjacent to food.”

“An unattended, unlocked housekeeping cart in the Secure Dementia Care Unit hallway contained ECOLAB Rapid Multi Surface Disinfectant Cleaner with a poison control warning label. Residents in this unit have been assessed as incapable of recognizing and using poisons safely.”

“The exhaust fan in room 214's bathroom was inoperable and the bathroom does not have an operable outside window, violating ventilation requirements.”

“Resident 1's bedroom contained an unlocked, unattended pill box with 14 pills accessible. Resident 2's bedroom contained unlocked, unattended medications including zinc oxide ointment, triple antibiotic ointment, refresh tears, tums, and fluocinonide .05% ointment.”

“Resident 2's room contained clotrimazole cream expired May 2016 and econazole nitrate 1% cream expired April 2017. The home does not have a current order for either medication. This is a repeated violation from 07/12/2022.”
