“Staff person A did not receive annual training in fire safety, emergency preparedness procedures, resident rights, and falls/accident prevention during 2024. Staff person B did not receive training in fire safety and emergency preparedness procedures. Staff person C did not receive training in fire safety, emergency preparedness procedures, and resident rights during 2024.”

“Emergency telephone numbers, including the nearest hospital and fire department, were not posted on or by the telephone in resident room 118.”

“The home failed to immediately notify the resident's designated person of an allegation of abuse, delaying notification until a later date.”

“Staff person A forcefully pulled a resident from bed, placed them on a commode, roughly put them back on bed, grabbed them by the wrists and pulled them up until noses were almost touching while screaming, threw the resident onto the bed, and called them "crazy." This abuse allegation was not reported to the local area agency on aging until a later date, failing to immediately report suspected abuse.”

“Staff person A was alleged to have abused a resident, but was not suspended or placed on an approved plan of supervision immediately upon the allegation being reported to the home.”

“An incident of abuse was witnessed by staff person B but was not reported to the Department within 24 hours, failing to timely report the incident in the manner designated by the Department.”

“Staff person A forcefully pulled a resident from bed, placed them on a commode, roughly handled them, grabbed them by the wrists and pulled them up until noses were almost touching while screaming, threw the resident onto the bed, and verbally abused them. The resident sustained bruises to both wrists. This is a repeat violation from prior inspections.”

“A resident prescribed a topical solution every 6 hours as needed for anxiety was being administered the medication to control behaviors, specifically to prevent the resident from refusing medication. The medication was administered when the resident exhibited spitting, throwing things, falling down, and cursing. This constitutes use of medication as a restraint.”

“The home validated the occurrence of multiple financial abuse incidents affecting all residents but failed to inform residents and their designated persons of the investigation findings as required.”

“A resident-home contract was not signed by the resident as required before or at the time of admission.”

“Multiple residents experienced financial abuse/exploitation including missing cash from rooms, fraudulent credit card charges, and unauthorized transactions. Six separate incidents of financial abuse were substantiated involving missing funds and fraudulent charges on residents' credit cards and debit cards.”

“A resident's preadmission screening form did not include a determination that the resident's needs could be met by the services provided by the home, as required within 30 days prior to admission.”
