“Based on record review and interview, for two of two residents sampled, the manager failed to ensure that a resident had a service plan that was established, documented, and implemented that was completed no later than 14 calendar days after the resident’s date of acceptance. Findings Include: 1. A review of R1’s medical record revealed R1’s initial service plan was dated May 19, 2022, years prior to R1’s date of admission to this facility. R1’s medical record’s revealed documentation labeled “Service Plan Update”, dated April 11, 2026, however, the service plan update did not include documentation of R1’s medical or health problems or the amount, type, and frequency of assisted living services and ancillary services being provided to the resident. 2. A review of R2’s medical record revealed R2’s initial service plan was dated February 21, 2022, years prior to R2’s date of admission to this facility. R2’s medical record’s revealed documentation labeled “Service Plan Update”, dated April 11, 2026, however, the service plan update did not include documentation of R2’s medical or health problems or the amount, type, and frequency of assisted living services and ancillary services being provided to the resident. 3. In an interview, E1 acknowledged R1 and R2’s initial service plans were not updated when the facility moved locations. 4. In an exit interview, the findings were reviewed with E1 and no further information was provided.”
