“Based on record review and interview, the manager failed to ensure that a resident's written service plan, when initially developed and when updated, was signed and dated by the resident or resident’s representative, the manager, and a nurse or medical practitioner for two of four residents sampled. Findings include: 1. A review of R2’s medical record revealed a service plan initiated on July 09, 2025 indicating R2 received personal care services. The service plan had a signature page which included the following signatures: - A nurse practitioner had signed and dated the service plan on July 16, 2025; - The facility manager had signed and dated the service plan on July 31, 2025; and - The resident’s representative had signed and dated the service plan on August 19, 2025. 2. A review of R3’s medical record revealed a service plan initiated on July 16, 2025, indicating R3 received directed care services, including medication administration. The service plan had a signature page which included the following signatures: - A nurse practitioner had signed and dated the service plan on July 23, 2025; - The facility manager had signed and dated the service plan on July 31, 2025; and - The resident’s representative had signed and dated the service plan on August 20, 2025. 3. In an exit interview, the findings were reviewed with E1. E1 advised that they send out the service plans to the residents representatives on time but sometimes do not receive them back. E1 stated they had no paper trail documenting their attempts to obtain signatures from the residents' representatives.”
