“Based on interview, record review, and documentation review, the manager failed to ensure a caregiver provided documentation of completion of a caregiver training program approved by the Department or the Board of Examiners for Nursing Care Institution Administrators and Assisted Living Facility Managers (NCIA Board), for one of two sampled caregivers. The deficient practice posed a risk if an employee was unqualified to provide caregiving services. 1. In an interview, E1 reported E2 was hired as a caregiver. 2. A review of E2's personnel record revealed E2 was hired as a caregiver in August 2014. The review revealed a caregiver certificate from "Ability Training and Services" (ALTP0173) dated June 30, 2013. 3. A review of the NCIA Board website revealed "Ability Training and Services" was active between January 26, 2011, and January 31, 2013. The review revealed "Ability Training and Services" was not an active caregiver training program on June 30, 2013, when the certificate was issued. 4. A review of facility documentation revealed a series of personnel schedules dated between January 2025 and February 2025. The schedules revealed E2 worked as a caregiver on a regular basis in January and February, 2025. 5. In an interview, E1 acknowledged "Ability Training and Services" was not an active training program when E2's certificate was issued.”
