“Based on documentation review, observation, record review, and interview, a person established, conducted, and maintained a health care institution (HCI) without a current and valid license issued by the Department. The deficient practice posed a risk as the unlicensed operation or maintenance of a health care institution is prohibited and is declared a nuisance inimical to the public health and safety, per A.R.S. § 36-430. Findings include: 1. A review of Department documentation revealed the license AL11681H was voided on December 17, 2025, as a result of non-payment of the annual license fee. 2. The Compliance Officer observed eight residents on-site at the time of the inspection conducted on March 9, 2026. 3. In an interview, E1 reported that all eight residents resided at the facility. 4. In an interview, E1 reported that E1 stored and administered medication to the residents. 5. A review of R1's, R2’s, R3’s, R4’s, R5’s, R6's, R7’s, and R8’s medical records revealed that medication administration records (MARs) and residents' MAR sheets showed documentation of receiving medication services from December 17, 2025, to March 9, 2026. 6. In an interview, E1 acknowledged that the facility was operating an HCI without a current and valid license issued by the Department after December 17, 2025.”
