“Based on record review and interview, the manager failed to ensure that a documented residency agreement included the policy and procedure for a resident to terminate residency, including terminating residency because of services not provided to the resident according to the resident’s service plan, for two of two residents sampled. The deficient practice posed a risk if the resident was not informed of the terms of residency   Findings include:   1. A review of R1's and R2’s medical records revealed a residency agreement; however, the residency agreements did not include the policy and procedure for a resident to terminate residency, including terminating residency because services were not provided to the resident according to the resident’s service plan. 2. In an interview, the findings were reviewed with E1, and no additional information was provided.”
