“Based on record review and interview, the manager failed to ensure that an employee possessed a valid fingerprint clearance card. E3's fingerprint clearance card was invalid at the time of the inspection. The deficient practice posed a risk if the employee was a danger to a vulnerable population.     Findings include:     1.     Record review established that E3 was a caregiver working at the facility.       2.     Record review established that E3’s fingerprint clearance card was invalid according to the Arizona Department of Public Safety database.   3.     The reason the clearance card was not valid was not available through the Arizona Department of Public Safety database.       4.     In an interview, E1 confirmed that E3 was a caregiver working at the facility. E1 also confirmed that E3’s fingerprint clearance card was invalid.”

The deficient practice posed a potential risk of explosion or leak of compressed gas.     Findings include:   1.  The Compliance Officer observed three unsecured oxygen tanks in R3’s room.   2.  In an interview, E1 confirmed that there were three unsecured oxygen tanks in R3’s room.”
