Based on observation and interview, the manager failed to ensure hot water temperatures were maintained between 95º F and 120º F in areas of an assisted living facility used by residents. The deficient practice posed a risk to the physical health and safety of a resident. Findings include:   1.   During an environment inspection, the Compliance Officers observed the water temperature at 133.9º F in the hall bathroom used by residents.   2. In an exit interview with E1, the findings were reviewed, and no additional information was provided.

Based on observation and interview, the manager failed to ensure that poisonous or toxic materials stored by the assisted living facility were inaccessible to residents. The deficient practice posed a risk to the physical health and safety of a resident. Findings include:   1.   During an environmental inspection of the facility, the Compliance Officers observed that the following material stored in a cabinet located under the kitchen sink area was accessible to the residents. -A bottle of concentrated bleach. The kitchen cabinet was equipped with a cable lock; however, the lock was not able to keep the cabinet door fully closed, leaving stored chemicals accessible to the residents.   2. During the environmental inspection, the Compliance Officers observed that the back of the kitchen cabinet located under the kitchen sink presented an open area that allowed the chemicals to be accessible to the residents.   3. In the exit interview, the findings were reviewed with E1, and no additional information was provided.
