“Based on record review and interview, the manager failed to ensure that a resident had a written service plan that was completed no later than 14 calendar days after the resident’s date of acceptance, for one of one residents sampled. The deficient practice posed a risk as there was no service plan to direct services to be provided to a resident. Findings include: A review of R1’s medical record did not include documentation of a completed service plan. Based on R1’s date of admission, this documentation was required. In an interview, E1 reported the facility’s nurse was unavailable to complete a service plan within the required time frame. E1 acknowledged R1's medical record did not include a service plan completed within 14 days of R1's acceptance.”
