“Based on record review and interview, the manager failed to ensure a medication administered to a resident was documented in the resident's medical record, for one of two residents sampled. Findings include: 1 . A review of R1's medical record revealed signed medication orders for the following medications: -Metoprolol Tartrate 50 MG, 1 tablet 2 times daily; -Potassium Chloride ER 10 MEQ once a day; and -Trazadone 50 MG once daily. However, the following medications were not documented as administered on the following days: -Metoprolol Tartrate (8 AM) from September 27, 2025 to September 30, 2025; (8 PM) from September 26, 2025 to September 30, 2025; -Potassium Chloride from September 27, 2025 to September 30, 2025; and -Trazodone from September 26, 2025 to September 30, 2025. 2 . In an exit interview, the findings were discussed with E1 and no additional information was provided.”

A review of facility documentation revealed documentation of rabies vaccinations for O1 and O2.
