“Based on observation and interview, the manger failed to ensure food was stored free from spoilage, filth, or other contamination and was safe for human consumption. The deficient practice posed a risk of food-borne illness. Findings include: 1. During the environmental inspection of the facility, the Compliance Officer observed a half a head of lettuce in an open plastic bag in the oven. The half a head of lettuce had multiple brown spots with a gray fuzzy substance. 2. In an interview, E1 acknowledged food was not stored free from spoilage, filth, or other contamination and may not have been safe for human consumption.”

In an interview, E1 acknowledged the bathroom accessible from the common area did not contain paper towels in a dispenser or a mechanical air hand dryer.”
