“Based on documentation review, record review, and interview, the manager failed to ensure that a caregiver's skills and knowledge were verified and documented before the caregiver provided physical health services, according to policies and procedures. Findings include: 1. A review of the facilities' policies and procedures revealed a document that state "The hiring person or manager will ensure, check and document that each caregiver or assistant caregiver providing physical health services or behavioral care services have the required skills and knowledge before providing any service. This will be completed by checking 2 Personal and 2 Professional references upon hire. Further, at ADRIANA'S ROUND-THE-CLOCK CARE HOME LLC, we seek those capable of patience, creativity, willingness to learn new techniques/program strategies and who are willing to look beyond the deficits to the humanity of the residents as related to their needs and are able to communicate effectively. The skills and knowledge are verified by manager at the time of the orientation, before providing assisted living services to a resident. and documented based on the care levels and type of assisted living services expected to be provided by the facility and within the facility's scope of service, in accordance to the residents needs and to ensure the health and safety of the resident. 2. A record review of E2's personnel record revealed no skills and knowledge verification. 3. In an interview, E1 acknowledged that E2 did not have skills and knowledge verified.”

Findings include: 1. A medical record review of R1 revealed that they require directed care services. 2. A record review of R1's medical record revealed a service plan that was dated April 19, 2025, and it listed the next review date as July 19, 2025. However, no updated service plan was in the resident's medical record. 3. A medical record review of R2 revealed that they require Personal Care services. 4. A record review of R2's medical record revealed a service plan that was dated January 9, 2025, and it listed the next review date as July 19, 2025. However, no updated service plan was in the resident's medical record. 5. In an interview, E1 acknowledged that service plans have not been updated according to the residents' level of care.”
