“Based on record review and interview, the manager failed to ensure a resident's medical record contained documentation of assisted living services provided to the resident, for two of two residents sampled. The deficient practice posed a risk if services provided to a resident could not be verified. Findings include: 1. A review of R1’s medical record revealed a current service plan dated March 23, 2026. The service plan indicated R1 received the following services: Clean room QD/PRN Laundry QD/PRN 2. A review of R1's medical record revealed an April 2026 activities of daily living (ADL) log. The ADL revealed no documentation of laundry services and room cleaning for R1. 3. A review of R2’s medical record revealed a current service plan dated February 22, 2026. The service plan revealed R2 received the following services: Assistance with dressing Hair care daily Oral care daily Apply skin barrier Rotate every 2-3 hours 4. A review of R2’s medical record revealed an ADL log for the month of February 2026. The following services were not documented from February 24, 2026 to February 28, 2026: Assistance with dressing Hair care daily Oral care daily Apply skin barrier Rotate every 2-3 hours 5. In an interview, E3 reported services were provided; however, they were not documented. 6. In an exit interview, the findings were reviewed with E1 and no additional information was provided.”
“Based on record review and interview, the manager failed to ensure a medication was administered in compliance with a medication order, for two of two residents sampled. The deficient practice posed a risk if the resident experienced a change in condition due to improper administration of medication. Findings include: 1. A review of R1’s medical record revealed a current service plan dated March 23, 2026. The service plan revealed R1 received medication administration. 2. A review of R1’s medical record revealed a medication order dated March 6, 2026. The order revealed “Fluticasone Propionate Nasal Suspension 50 MCG/ ACT 2 spray in both nostrils two times a day for allergies.” 3. A review of R1’s medical records revealed an April 2026 medication administration record (MAR). The MAR revealed Fluticasone 50 MCG was being marked with a “/”. 4. In an interview, E4 reported “/” means the medication was not administered because it is PRN [as needed]. E4 was not aware Fluticasone 50 MCG was scheduled. 5. A review of R2’s medical record revealed a current service plan dated February 22, 2026. The service plan revealed R2 received medication administration. 6. A review of R2’s medical record revealed a medication order dated February 6, 2026. The order revealed “Metoprolol 25 mg ½ tab bid hold if heart rate is below 55”. 7. A review of R2’s medical record revealed an April 2026 MAR. The MAR revealed Metoprolol 25 mg was administered April 1, 2026 8 am and 5pm to April 6, 2026 am. 8. A review of R2’s medical record revealed no documentation of heart rate being recorded and no documentation was provided at the time of the inspection. 9. In an exit interview, the findings were reviewed with E1 and no additional information was provided.”

The deficient practice posed a risk as medication could not be verified as administered against a medication order. Findings include: 1. A review of R1’s medical record revealed a current service plan dated March 23, 2026. The service plan revealed R1 received medication administration. 2. A review of R1’s medical record revealed medication orders for the following medications: Buspirone HCI Oral tablet 5 mg give 1 tablet by mouth two times a day, Donepezil HCI oral tablet 10 MG give 1 tablet by mouth at bedtime, Gabapentin Oral capsule 100 MG give 1 capsule by mouth three times a day, Trazadone HCI oral tablet 100 MG give 1 tablet by mouth at bedtime, Senna Lax 8.6 MG give 2 times a day, Januvia 100 mg tablet 1 time a day, Loratadine 10 mg tablet 1 time a day, and Rosuvastatin 10 mg tablet at bedtime. 3. A review of R1’s medical record revealed an April 2026 medication administration record (MAR). The following medications were not documented as administered on April 6, 2026: Buspirone HCI Oral tablet 5 mg at 5 pm, Donepezil HCI oral tablet 10 mg at 5 pm, Gabapentin Oral capsule 100 mg at 2 pm and 8pm, Trazadone HCI oral tablet 100 mg at 8 pm, and Senna Lax 8.6 MG at 5 pm The following medications were not documented on the April 2026 MAR: Januvia 100 mg tablet 1 time a day, Loratadine 10 mg tablet 1 time a day, and Rosuvastatin 10 mg tablet at bedtime. 4. A review of R2’s medical record revealed a current service plan dated February 22, 2026. The service plan revealed R2 received medication administration. 5. A review of R2’s medical record revealed medication orders for the following medications: Eliquis 5 mg 1 tab BID, Hydralazine 25 mg 1 tab TID, and Metoprolol 25 mg ½ tab bid hold if heart rate is below 55. 6. A review of R2’s medical record revealed an April 2026 MAR. The following medications were not documented as administered on April 6, 2026: Hydralazine 25 mg at 2 and 8 pm Metoprolol 25 mg at Eliquis 5 mg was documented as administered only at 8am on April 1, 2026. 7. In an exit interview, the findings were reviewed with E1 and no additional information was provided.”
“Based on observation and interview, the manager failed to ensure that medications stored by the assisted living facility were stored in a separate locked room, closet, cabinet, or self-contained unit used only for medication storage. The deficient practice posed a health and safety risk if medication was accessible to residents. Findings Included: 1. The Compliance Officers observed an unlocked caregiver’s room. Inside the unlocked caregiver’s room the following medications were found: Hydroco/APAP 5-325 MG Cephalexin 500 MG Claritin 10 mg 2. In an interview, E4 reported the medications were for residents. 3. In an exit interview, the findings were reviewed with E1 and no additional information was provided.”
