“Based on documentation review, record review and interview, the manager failed to ensure a caregiver provided a resident with the assisted living services in the resident's service plan and documented the services provided in the resident's medical record. Findings include: 1. A review of the facility's policies and procedures revealed a page titled "Service Plans", which stated: "The facility staff shall provide care services as specified on the service plan. The services provided shall be documented on the "Services Provided" form for each resident". 2. A review of R2's medical records revealed a service plan that showed R2 would need blood sugar checks done by staff twice a day. Further review revealed R2's medical record contained no mention or documentation of blood sugar checks. 3. In an interview, the findings were reviewed with E1, and no additional documentation was provided.”
