“The provider was out of compliance with this rule by failing to ensure each resident was free from mistreatment. During the inspection, there were no interventions to protect a resident after her husband had forcefully placed a spoon full of pills into her mouth in November 2024. In a separate incident that took place in February 2025, the husband raised his voice to his wife, shoved her and removed her dress.”
