The secure unit lacked a written preventative maintenance schedule, and multiple maintenance issues were not documented in work orders, including a broken fire exit door that did not latch properly (with a piano placed in front of it), improperly vented dryer, malfunctioning exit keypad, and use of extension cords. These findings indicate the facility did not ensure the building systems and safety equipment were properly maintained and repaired.
“The provider was out of compliance with R432-270-24(1) by not ensuring the preventative maintenance was conducted according to a written schedule to ensure that the facility was safe and in good repair. During the investigation, it was observed in the secure unit that there was no written preventative maintenance schedule and several maintenance tasks were not completed. It was also it observed that 1 fire exit door was broken and closed, but did not latch. There was a piano placed in front of the door to deter residents from attempting to open the door.”
