“Resident personal equipment (bedside mobility devices) were not properly secured to beds and created entrapment hazards. Three residents had unsecured u-shaped bedside mobility devices that could slide out easily, creating approximately 1 foot entrapment zones between the devices and mattresses. One device was covered with a loose manufacturer-unapproved pillowcase.”

“Poisonous materials (Secret deodorant with poison control label) were left unlocked, unattended, and accessible to a memory care resident who has not been assessed as capable of safely using or avoiding poisonous materials. This is a repeat violation from 07/08/2024 and 04/24/2024.”

“Trash outside the home was not properly contained. The ground around the fenced-in dumpster area was covered almost entirely in loose garbage including disposable cups and take-out containers. This is a repeat violation from 3/19/2024.”

“Four cats were present at the home; however, current rabies vaccination certificates were not available for two of the cats. The rabies vaccinations for these two cats had expired on 6/13/2024 and 1/27/2025.”

“Emergency procedures were not posted in a conspicuous and public place in the home. The red binder containing the home's emergency procedures was found in a drawer behind the front desk on 4/23/2025.”

“Loose pills were observed in medication carts. Two white oblong pills, one round white pill, and one orange round pill were found in the memory care medication cart, and one additional loose white oblong pill was observed in the second-floor medication cart.”

“The home failed to report to the Department within 24 hours an incident in which a resident was sent to the hospital due to an accident and subsequently passed away.”

“A resident-home contract was not signed by the resident's designated person. The contract indicated the resident refused to sign but did not document that the resident was given the opportunity to sign.”

“A direct care staff person did not receive orientation on their first work day covering evacuation procedures, staff duties during fire drills and emergency evacuation, designated meeting places, smoking safety procedures, fire extinguisher location and use, smoke detectors and fire alarms, and telephone use for emergency services.”

“A resident's prescription medication container did not have a pharmacy label including the resident's name, medication name, prescription issue date, prescribed dosage and administration instructions, and prescriber name and title. Additionally, a direction change from twice daily to once daily at bedtime was not indicated on the medication container.”

“The facility did not follow the prescriber's orders for a resident's medication. A prescribed medication was administered at an incorrect time, and another prescribed medication was not administered on two separate occasions. Additionally, the resident was not properly documented as being on leave of absence (LOA) on dates when the medication was not given.”
