Three residents had unsecured u-shaped bedside mobility devices that could slide out easily, creating approximately 1 foot entrapment zones between the devices and mattresses. One device was covered with a loose manufacturer-unapproved pillowcase.”
“Poisonous materials (Secret deodorant with poison control label) were left unlocked, unattended, and accessible to a memory care resident who has not been assessed as capable of safely using or avoiding poisonous materials. This is a repeat violation from 07/08/2024 and 04/24/2024.”
