“Three insulin pens (Novolog Flex Pen for Residents #4 and #5, and Lantus Solostar for Resident #5) were opened and undated, contrary to manufacturer's instructions requiring dating upon opening and expiration after 28 days.”

“Two residents' medications had pharmacy labels with incorrect dosing instructions: Resident #6's Ondansetron labeled as every 6 hours but prescribed every 8 hours, and Resident #5's Lantus Solostar labeled as 14 units but prescribed as 10 units.”

“Two residents' glucometers were not set to the correct time and date (Resident #4 showed 2:38 pm on 12/26/2025 instead of current time, Resident #5 showed 2:26 pm on 12/29/2026 instead of current time), and Resident #6's prescribed Lorazepam 2 mg/ml oral concentrate was not available in the home.”

“Battery-operated carbon monoxide detectors in multiple areas (Boathouse, Cottage, Garden, and Country areas) were missing required dates of battery installation as mandated by the Care Facility Carbon Monoxide Alarms Standards Act. Batteries must be dated and replaced annually.”

“A dose of Morphine Sulfate 20mg/1ml (0.5ml) administered to Resident #2 at 1:30am on 12/3/24 was documented in the controlled substance record but was not recorded on the resident's December 2024 medication administration record (MAR). Medication administration must be recorded at the time of administration.”

“A resident intentionally tipped another resident's wheelchair over, causing the resident to fall and hit their head on the wall, then attempted to drop the wheelchair on top of the fallen resident. Both residents were transported to the hospital. This was a repeat violation from 01/08/2024.”

“Alleged verbal abuse of a resident by direct care staff person B was not immediately reported to the Department of Aging in accordance with the Older Adults Protective Services Act. The incident was witnessed by direct care staff person A but reporting was delayed.”

“Direct care staff person B, alleged to have verbally abused a resident, was not immediately suspended or placed under a supervision plan and continued to work unsupervised for multiple shifts following the alleged incident.”

“The allegation of verbal abuse was not reported to the Department's personal care home regional office or complaint hotline within 24 hours as required.”

“A resident was not treated with dignity and respect when direct care staff person B crouched down, got in the resident's face, and made disrespectful comments about the resident's husband, which was characterized by another staff member as disgusting and very disrespectful.”

“A resident was physically abused by another resident who grabbed them by the neck, pulled them into a bedroom, and caused bruising and red marks. Staff person B was also physically assaulted when the resident smacked them across the face and pushed them by the head and neck.”
