“On the inspection date at approximately the specified time, the medication cart beside the laundry room on the third floor was unlocked, unattended, and accessible.”

“Medications prescribed for ten days for Residents #1, #2 and #3 were found in the residence's first floor medication cart on 3/28/24; however, these medications were discontinued prior to the inspection date and should not have been in the cart.”

A resident was observed with a bottle of cleaning solution pressed to their lips, and multiple poisonous items including cleaning solutions, ointments, shampoos, and body wash products were found in unlocked drawers in the resident's bathroom.”
