“A resident wandered into another resident's room, causing agitation. The resident in the room attempted to strike the wandering resident with a cane, then took the cane and struck the wandering resident several times, resulting in multiple skin tears on knees, elbows, and right hand (ranging 2-4 cm in the knuckle area).”

“Resident #1 admitted to the Secure Dementia Care Unit on 8/25/23, but the written cognitive preadmission screening was not completed within the required 72 hours prior to admission. The screening was completed after admission due to delays in obtaining a new DME.”

“Directions for operating the electronic door locking mechanism are not conspicuously posted near or at doors that exit the Secure Dementia Care Unit. The facility uses an electronic fob system to control egress, but proper operational instructions were not posted.”
