The resident's support plan did not document the resident's history of inappropriate comments or this incident of inappropriate touching.”

“Resident's support plan did not include mention of the resident engaging in inappropriate comments or the recent incident of inappropriate touching that occurred on 8/30/2025. The support plan was not revised to reflect the significant change in the resident's condition and behavior.”
