“A resident in the Secure Dementia Unit approached another seated resident and bilaterally grabbed and squeezed the resident. Staff immediately separated the residents. The resident's support plan did not document the resident's history of inappropriate comments or this incident of inappropriate touching.”

“Resident's support plan did not include mention of the resident engaging in inappropriate comments or the recent incident of inappropriate touching that occurred on 8/30/2025. The support plan was not revised to reflect the significant change in the resident's condition and behavior.”

“Resident #4 self-administers some medications, but the resident's assessment and support plan and Document of Medical Evaluation both state the resident is unable to self-administer medications.”
