Based on observation, record review, documentation review, and interview, for two of two residents reviewed, who received a controlled substance, the manager failed to ensure that policies and procedures were implemented for inventorying controlled substances. The deficient practice posed a risk if controlled substances were not inventoried and accounted for by the facility.   Findings include:   1. In observation, R1 and R2 had Lorazepam medication (a schedule IV-controlled substance) on site and stored by the facility. 2. In record review, R1's (received personal care and medication administration services) medication administration record (MAR), dated March 2025, included documentation R1 received Lorazepam medication on March 24, 25, and 26th, 2025. R2's (received personal care and medication administration services) MAR dated March 2025, included documentation R2 received Lorazepam medication daily on March 12, through March 28, 2025. Neither record included an inventory of the Lorazepam medication.   3. In documentation review, a facility policy, titled "Medications... documented, "... 4. The opioid and narcotic medication will be inventoried and placed in the medication storage area. 5. Daily narcotics or controlled substance administration will be recorded on each resident Narcotic Inventory Sheet... 4. During an interview, E1 acknowledged the facility did not maintain an inventory of a controlled substance, in adherence to the facility's policy and procedures.
