Based on documentation review and record review, the manager failed to ensure a caregiver's or assistant caregiver's skills and knowledge were verified and documented before the caregiver or assistant caregiver provided physical health services and according to policies and procedures, for one of two personnel sampled. The deficient practice posed a risk if the employees were unable to meet a resident's needs. Findings include: 1 . A review of facility documentation revealed a policy titled "Employees and Volunteers Qualifications." The policy stated, "The hiring individual will check and document qualification, skills and knowledge for each employee...documentation of such check is going to be kept in the employee's record upon hiring ("Employee Orientation" and "Employee Qualifications and Skills.") 2 . A review of E2's personnel record revealed documentation of "Employee Qualifications and Skills" was not available for review at the time of inspection. 3 . In an interview, E1 acknowledged E2 had no skills and knowledge verified and documented before providing services and according to policies and procedures.

Based on documentation review and interview, the manager failed to ensure a pest control program that complied with A.A.C. R3-8-201(C)(4) was implemented and documented. Findings include: 1 . During an inspection, the Compliance Officer requested to review the facility's pest control program. Documentation of the facility's pest control program was not available for review at the time of inspection. 2 . In an interview, E1 reported E1 had been spraying their own facility, and E1 reported E1 had no certification. E1 acknowledged E1 had no pest control program that complies with A.A.C. R3-8-20l(C)(4) was implemented and documented.
