“Based on observation and interview, the manager failed to ensure foods requiring refrigeration were maintained at 41\'b0 F or below. The deficient practice posed a health risk to the residents. Findings include: 1. During the facility tour, the Compliance Officer observed an open container of "Welch's strawberry spread" and "Kroger Original pancake syrup" in a kitchen cabinet. Both of these containers stated "Refrigerate after opening." 2. During an interview, E1 acknowledged the foods were stored at room temperature and required refrigeration.”

“Based on observation and interview, the manager failed to ensure poisonous or toxic materials stored by the assisted living facility were maintained in a locked area inaccessible to residents. The deficient practice posed a risk to the physical health and safety of a resident. Findings include: 1. During an environmental inspection of the facility, the Compliance Officer observed ambulatory residents. 2. During an environmental inspection of the facility, the Compliance Officer observed "All Clear Jumbo Chlorinating tablets" behind an unlocked gate in the back yard. 3. In an interview, E1 reported the toxic chemical had been left there by the pool cleaner. E1 acknowledged poisonous or toxic materials stored by the assisted living facility were not maintained in a locked area inaccessible to residents.”
