Findings include: 1 . A review of E2's personnel record revealed documentation of fall prevention and fall recovery training was not available for review at the time of inspection. 2 . In an interview, E1 reported E1 was unaware E2 needed fall prevention and fall recovery training. 3 . In an exit interview, the findings were discussed with E1 and no additional information was provided.”
