“Based on record review and interview, the manager failed to ensure that an emergency response document was available for each resident residing in the facility which contained the required information for A.R.S. 36-420.04.A.1-9. Findings include: 1 . A review of R1's records revealed no Emergency Response form to be provided to emergency personnel in the event that an emergency responder was contacted. 2 . In an exit interview, E1 acknowledged that there was not an Emergency Response document for all members in the facility.”

“Based on record review, document review, and interview, the health care institution failed to ensure that annual training and education related to recognizing the signs and symptoms of tuberculosis to an individual employed and annually assessing the facility's risk of exposure to infectious tuberculosis was completed per R9-10-113.A,1-2. Findings include: 1 . A review of E1's and E2's employee files revealed no training or education provided related to recognizing the signs and symptoms of tuberculosis. 2 . A review of facility documents revealed no annual assessment of the facility for the risk of exposure to infectious tuberculosis. 3 . In an exit interview, the findings were reviewed with E1 and no further information was provided.”

“Based on record review and interview, the manager failed to ensure that before providing assisted living services to a resident, a manager or caregiver provided current documentation of first aid training and cardiopulmonary resuscitation training certification specific to adults. The deficient practice posed a risk if an employee was unable to meet a resident's needs during an emergency. Findings include: 1 . During a review of E1's records, the Compliance Officer found a Cardiopulmonary Resuscitation and First Aid certificate, however, the certificate was expired, dated July 15, 2025. 2 . In an exit interview, the findings were reviewed with E1 and no further information was provided.”
