“Based on documentation review, record review, and interview, the administrator failed to ensure the health care institution developed and administered a training program for all staff regarding fall prevention and fall recovery. Findings include: 1. A review of facility documentation revealed no evidence of an established fall prevention and fall recovery training program. 2. A review of E3's personnel record revealed no documentation to indicate E3 completed fall prevention and fall recovery training. 3. In an interview, E1 acknowledged E3's personnel record contained no documentation to indicate E3 completed fall prevention and fall recovery training.”

“Based on record review and interview, the manager failed to ensure a personnel record for an employee included documentation of current cardiopulmonary resuscitation (CPR) and first aid training, for one of five sampled caregivers. Findings include: 1. A review of E2's personnel record revealed E2 was hired as a caregiver. E2's personnel record contained documentation of CPR and first aid certification. However, the certification expired October 1, 2023. 2. In an interview, E1 acknowledged E2's first aid and CPR training certifications were expired, and there was no additional documentation available for review.”
