“Based on record review and interview the health care institution failed to develop and administer a training program for all staff regarding fall prevention and fall recovery as required in A.R.S. \'a7 36-420.01. Findings include: 1. Review of the record for E1 (hired August 26, 2013), revealed that fall prevention and fall recovery continued competency training had last been conducted on June 27, 2022. 2. Review of the record for E2 (hired January 19, 2019), revealed that fall prevention and fall recovery continued competency training had last been conducted on May 10, 2023. 3. Review of the facility policy and procedure for fall prevention and fall recovery continued competency training indicated that training would be conducted "..upon hire and at least every 12 months thereafter.". 4. During an interview, E1 acknowledged that continued competency training for fall prevention and fall recovery had not been conducted as specified in policy and procedure.”
“Based on documentation review and interview, the manager failed to ensure that a food menu is prepared at least one week in advance. Findings include: 1. Sixty days of menus were requested. Review of the menus revealed the menu dated November 18 - 24, 2024 lacked an identification of the menu items to be served for dinner. 2. During an interview, E1 stated "I know what we are having I just haven't written it down yet." 3. During an interview, E1 acknowledged the required documentation was not available for review.”
