“Based on record review and interview, the manager failed to ensure that the facility was in compliance with R9-10-808.A.4.b.i-iii. The deficient practice posed a risk as there was no service plan to direct services to be provided to one of two residents sampled.   Findings include:   1.  Record review established that R1 was at the personal level of care and had an outdated service plan, which was completed on June 30, 2024.   2.  In an interview, E1 confirmed that R1 was at the personal level of care and R1’s service plan needed to be updated every six months. E1 confirmed that the last update to R1’s service plan was on June 30, 2025.”

“Based on documentation review and interview, the manager failed to ensure a disaster drill for employees was conducted on each shift at least once every three months and documented. The deficient practice posed a risk if employees were unable to implement a disaster plan.     Findings include:   1.  Documentation review established that the facility conducted its last disaster drill on December 4, 2024.   2.  In an interview, E1 confirmed that the facility conducted its last disaster drill on December 4, 2024.”
