“Based on record review and interview, the manager failed to ensure a caregiver provided assistance with activities of daily living according to the resident's service plan, for one of two residents sampled. Findings include: R9-10-101.5. "Activities of daily living" means ambulating, bathing, toileting, grooming, eating, and getting in or out of a bed or a chair. A.R.S. \'a7 36-401.16. "Directed care services" means programs and services, including supervisory and personal care services, that are provided to persons who are incapable of recognizing danger, summoning assistance, expressing need or making basic care decisions. 1. A review of R1's medical record revealed a current service plan for directed care dated in January 2024. The service plan revealed R1 was to receive assistance with showering 2-3 times per week and combing hair 2 times daily. 2. A review of R1's medical record revealed a "Vitals and Activities of Daily Livings" document dated in February 2024. The document indicated R1 received assistance with the following services on the following dates: -February 5, 2024 (shower); and -February 14, 2024 (shower). However, documentation R1 received assistance with showering 2-3 times per week and combing hair 2 times daily, February 1-14, 2024 was not available for review. 3. In an interview, E2 reported R1 received assistance with one shower a week and a partial bath when requested. 4. In an interview, E2 acknowledged the services had not been provided according to the service plan.”
