“Based on record review and interview, for one of three residents reviewed, the manager failed to ensure a medical record included all required information for the resident. Findings include: 1. During the environmental inspection of the facility, the Compliance Officer requested R1's, R2's, and R3's medical records; however, a medical record for R1 was unavailable for review. 2. In an interview, E2 reported R1 was admitted to the facility on April 14, 2024, and left the facility on May 12. 2024. 3. In an interview, E1 and E2 acknowledged there was no medical record for review for R1.”
