“Based upon observation and interview, the manager failed to ensure that the alarms were engaged for the front and back doors of the facility. The deficient practice posed a risk if the facility was unaware of the general or specific whereabouts of a resident. Finding include: 1. The Compliance Officer observed that the front and back doors of the facility were alarmed and the alarms for both doors were not engaged. 2. In an interview, E1 confirmed that the alarms to the front and back doors of the facility were not engaged.”

The deficient practice posed a risk to the physical health and safety of a resident. Findings include: 1. The Compliance Observed the following unsecured toxic chemicals underneath the kitchen sink of the facility: - Lysol All Purpose Cleaner - Bar Keepers Friend Cleanser 2. In an interview, E1 confirmed that the Lysol All Purpose Cleaner and the Bar Keepers Friend Cleanser were both underneath the kitchen sink of the facility and unsecured.”
