“Based on record review, documentation review, and interview, the health care institution failed to implement tuberculosis (TB) infection control activities, including annually providing training and education related to recognizing the signs and symptoms of TB to individuals employed by the health care institution for two of three personnel sampled, and annually assessing the health care institution's risk of exposure to infectious tuberculosis. The deficient practice posed a risk as the caregiver received no organized instruction or information related to TB surveillance. Findings include: 1. A review of E1's personnel record revealed no documentation of training and education related to recognizing the signs and symptoms of TB. Based on E1's date of hire, this documentation was required.   2. A review of E2's personnel record revealed no documentation of training and education related to recognizing the signs and symptoms of TB. Based on E2's date of hire, this documentation was required. 3. In an interview, E1 acknowledged that E1 and E2 did not have the annual TB signs and symptoms training.   4. In an exit interview, the findings were reviewed with E2 and E4, and no additional information was provided.”

“Based on record review, documentation review, and interview, the manager failed to ensure that, before providing assisted living services to a resident, a caregiver provided current cardiopulmonary resuscitation (CPR) training specific to adults, for one of three personnel sampled. The deficient practice posed a risk if an employee was unable to meet a resident's needs during an emergency. Findings include:  1. A review of E3's personnel record revealed a certificate of completion for CPR and Automated External Defibrillator (AED) from the American Healthcare Academy with an expiration date of January 23, 2028.  2. A review of the facility’s personnel schedule for March 2026 revealed that E3 worked every Monday to Saturday in March.  3. A review of the American Healthcare Academy website, https://cpraedcourse.com/course/cpr-first-aid-certification/1/ revealed that the website stated: “Our online CPR/AED First Aid certification combo course includes Adult, Child and Infant CPR and First Aid training.”  4. A review of the facility’s policies and procedures revealed a policy titled “Cardiopulmonary Resuscitation (CPR) and First Aid Requirements.” The policy stated, “Each manager, caregiver, and other applicable employees shall: obtain CPR training specific to adults which includes a demonstration of the individual’s ability to perform CPR. (Online programs do not meet this requirement unless they include a demonstration of the individual’s ability to perform CPR.”  5. In an exit interview, the findings were reviewed with E2 and E4, and no additional information was provided.”

“Based on record review and interview, the manager failed to ensure that a resident's medical record contained a medication order from a medical practitioner for each medication that was administered to the resident, for one of two residents sampled. The deficient practice posed a risk as the medication administered could not be verified against a medication order. Findings Include: 1. A review of R1's medical record revealed the following:  A current written service plan dated March 18, 2026. This service plan indicated R1 received medication administration.   A medication list dated March 18, 2026. However, the medication list was not signed by a medical practitioner.  Medication Administration Record (MAR) for March 2025, signed that medications were administered.  2. A review of R1’s medication administration record (MAR) revealed “Lorazepam 1 mg - take 1 tab/PO/at bedtime for anxiety.”  3. A review of R1’s medical record revealed no signed order for “Lorazepam 1 mg.”  4. In an interview, E4 acknowledged that the medication was administered without a signed medication order.  5. In an interview, E4 reported that medications were administered to R1 and that the medication lists were not signed by a medical practitioner.  6. In an exit interview, the findings were reviewed with E2 and E4, and no additional information was provided.”
