§ 05 · Tour Prep

Questions to ask before you visit.

A short pre-tour checklist tailored to High Point Village's record and state requirements.

01 /

High Point Village holds a DSHS Specialized Dementia Care contract — can you provide a written copy of your dementia care program and explain how it meets the contract requirements for specialized supports?

Ask the operator on tour. Take notes and compare answers across facilities you visit.

02 /

The most recent DSHS inspection on June 1, 2025 resulted in one deficiency — what was the nature of that deficiency, and can you show us the corrective action plan your facility submitted to address it?

Ask the operator on tour. Take notes and compare answers across facilities you visit.

03 /

Since your community is licensed for 75 beds and holds a specialized dementia care contract, how do you organize residents with dementia — are they in dedicated units or integrated, and what policies guide staffing and activity programming for those residents?

Ask the operator on tour. Take notes and compare answers across facilities you visit.

Dependents of a service member are the service member’s spouse, the service member’s minor child, or and individual for whom the service member provided more than one-half of the individual’s support for one hundred eight days immediately preceding an application for relief.
