“Staff Member A was heard and observed yelling obscenities regarding care for Resident 1, referring loudly in the presence of other residents that Resident 1 smells like urine, using coarse language, and refusing to provide care. On a separate occasion, Staff Member A was observed arguing with Resident 2 and telling them to be quiet and sit without explanation.”
