At least one certified staff member is required for every 50 residents at all times.”

“Five unlabeled, used bars of soap were found in second-floor shared bathroom. Bar soap is prohibited unless clearly labeled for each resident who shares the bathroom; soap dispensers with liquid soap are required.”

“Cardboard box of air filters was stored near gas-powered hot water heater in furnace room #3, violating the prohibition on storing combustible and flammable materials near heat sources or hot water heaters.”

“A staff member who is not a licensed staff member administered prescription medication injections (GLP-1 agonist) to a resident without a current waiver authorizing this staff member to administer such medications.”

“Multiple doses of prescribed VA medications were not administered to a resident on several dates because the medications were not available in the home, indicating the facility did not follow the prescriber's orders.”
