The pathway is elevated approximately one step above surrounding ground, and the handrail on the ramp covers only a portion of the ramp. The pathway and ramp need the railing to be extended from the door to the driveway.”

“Approximately five fire extinguishers (2 in the dining room, 1 each on the Memory Care patio, in the TV Room, and by Fire Tower F) that were replaced following an annual fire extinguisher inspection did not have an inspection sticker or current inspection date.”

“Uncovered garbage cans full of garbage were found in an unlocked trash room on the first floor, creating an unsanitary condition.”

“The door to the attic in the stairwell next to the activities room was open with bags of insulation materials placed precariously near the opening, creating a hazard.”

“Resident #1's medical evaluation did not indicate the resident's ability to self-administer medications, missing a required component of the medical evaluation form.”

“Resident #1, who was self-administering medications, was unable to identify which pills were which or the purpose/diagnosis for each medication, indicating the resident was not capable to self-administer.”

“Resident #1's medication record did not include a current list of all medications; the record was missing current prescriptions for 50-mcg vitamin D capsules, 20-mg Furosemide tablets, 500-mg Acetaminophen, and a PRN prescription for MiraLAX.”

“Blood glucose readings for Resident #2 were not logged on the medication administration record in a timely manner; readings taken at specific times were documented hours later (e.g., reading at 7:11 am logged at 9:35 am, reading at 4:49 pm logged at 6:28 pm), and one reading of 559 at 7:20 pm on 4/1/2025 was not logged at all.”

“Medication administration records for Residents #2, #3, and #4 did not indicate the diagnoses or purposes for their prescribed medications, missing required documentation.”

“The ice cream freezer temperature was measured at 2 degrees Fahrenheit instead of the required below 0°F. Additionally, an opened jar of capers requiring refrigeration after opening was found stored in dry food storage instead of the refrigerator.”

“No signage was posted to indicate smoking was prohibited or designated to specific areas only, as required by the Clean Indoor Air Act regulations. The home allows smoking in designated areas but failed to post required notices.”

“Sanitary conditions were not maintained in the memory care kitchen refrigerator, which contained crumbs, spills, food particles, debris, and splashed food particles and liquids in the bins. Additionally, the gas grill in the main kitchen was covered with old food particles and excessive grime and buildup.”

“The bathroom sink in Resident 1's room was clogged and would not drain, indicating equipment that was not in good repair and free of hazards.”

“The first aid kit in the personal care nurse station did not include a breathing shield, which is a required component of a complete first aid kit.”

“Multiple 5-gallon emergency water bottles were stored on the floor in the shed located outside the building instead of on elevated storage. This was a repeat violation from a prior inspection date of 3/6/23.”

“A bag of toasted pearl couscous in the dry food storage area of the main kitchen was opened and unsealed, violating the requirement that food be stored in closed or sealed containers.”

“Two white laundry baskets full of clothes in the first-floor laundry room were not labeled with residents' names or room numbers, creating a risk that clothing could be lost or misplaced during laundering.”

“A cat present in a resident's room did not have a current certificate of rabies vaccination from a licensed veterinarian; the rabies certificate on file had expired on 5/8/24.”
