“Resident #1, who was self-administering medications, was unable to identify which pills were which or the purpose/diagnosis for each medication, indicating the resident was not capable to self-administer.”

“Resident #1's medication record did not include a current list of all medications; the record was missing current prescriptions for 50-mcg vitamin D capsules, 20-mg Furosemide tablets, 500-mg Acetaminophen, and a PRN prescription for MiraLAX.”

“A cat present in a resident's room did not have a current certificate of rabies vaccination from a licensed veterinarian; the rabies certificate on file had expired on 5/8/24.”

“Staff persons A and B did not have a record of training that included the trainer, date, source, content, length of each course, and copies of any certificates received.”
